It is for English-taught degrees, which is why an Indian student can sit it without knowing a word of Italian. And it is an entrance test, so your score is the admission decision.

There is no interview, no statement of purpose, no portfolio and no separate university application form at the selection stage. You register, you sit a 100-minute paper, you get a number, and that number places you in a ranking. For students used to the Indian system, this works far more like NEET than like a UK or US application.
The exam runs once a year, in late September, at test centres in Italy and in a list of countries abroad that includes India.

IMAT Full Form and What “International” Actually Means
IMAT stands for the International Medical Admissions Test.
The word "international" is the part students misread. It does not mean the test is run by an international body, and it does not mean the exam is only for foreigners. It refers to the degrees: these are the internationally oriented, English-taught versions of Italian medical courses, open to Italian and foreign applicants alike. An Italian student from Bologna and an Indian student from Bengaluru sit the same paper on the same afternoon.
Italy runs a parallel admission route for its Italian-taught medical degrees. The IMAT exam is the English-language track, and the two systems have diverged in recent years, which is why news about Italy reforming medical admissions often does not apply to the courses Indian students are looking at.
One clarification worth making early. In India, searching "IMAT" also returns a design and fashion aptitude test conducted by a Delhi institute. It is an entirely unrelated exam. Every year a handful of students prepare for the wrong one. Check the full form before you pay for anything.
Who Conducts the IMAT Exam
The IMAT exam is conducted by MUR, Italy's Ministry of University and Research.
This is where most online sources get it wrong, so it is worth setting out the actual chain of custody.

Four bodies touch the exam today, and knowing which does what saves you writing to the wrong one:
- MUR sets the rules, publishes the annual decree, fixes the date and syllabus, and publishes the rankings.
- CINECA, the ministry's inter-university consortium, prepares the questions under the 2026 decree.
- Universitaly is the government portal where you register and pay. There is no other registration route.
- The host university runs the physical test venue and receives a share of your registration fee as a logistics reimbursement.
How stale is the Cambridge claim? The University of Milan's own official IMAT page still describes the test as designed by Cambridge Assessment, four years after Cambridge left. When a participating university has not updated its page, every Indian aggregator copying from it repeats the error too.
Counsellor insight: Use the Cambridge line as a freshness test. If a coaching site or agent page still credits Cambridge Assessment, assume its dates, fees and seat numbers are equally out of date, and verify everything elsewhere.
What the IMAT Exam is Used For
The IMAT exam gives access to three English-taught degree programs at Italian universities.
- Medicine and Surgery, degree class LM-41, a six-year single-cycle program. This is what Indian students usually mean by MBBS in Italy.
- Dentistry and Dental Prosthodontics, degree class LM-46.
- Veterinary Medicine, degree class LM-42.
At the moment you register, you must declare which track you are competing for: medicine plus dentistry together, or veterinary medicine. That choice becomes irrevocable when registration closes, and you cannot compete for both.
One point that causes real confusion in 2026. Italy has reformed admission to its Italian-taught medical degrees through an open-semester system, and headlines about Italy scrapping its medicine entrance test have travelled widely. The current decree explicitly excludes English-taught courses from that reform. If you are applying in English, the IMAT exam still applies exactly as before.
Around twenty Italian public universities take part, alongside some private institutions, spread from Milan and Bologna in the north to Messina and Catania in the south. What IMAT Exam Looks Like
The IMAT exam is a paper-based test of 60 multiple-choice questions to be answered in 100 minutes.
Each question has five options with one correct answer. No calculator is permitted, so arithmetic has to be done by hand. The paper covers reading skills and general knowledge, logical reasoning, biology, chemistry, and physics with mathematics, with biology carrying the largest share by some distance.
Scoring is 1.5 marks for a correct answer, minus 0.4 for a wrong one, and zero for a blank, out of a maximum of 90. The negative marking matters more than students expect: with five options, guessing at random loses you marks on average.

Can Indian students take the IMAT Exam
Yes. India is one of the countries listed in the current decree as hosting an overseas test centre, so you can sit the exam without travelling to Italy.
Three things an Indian applicant should know before going further:
- Eligibility is simple. You need a completed Class 12 qualification to enrol, and registration on the Universitaly portal within the annual window. No Italian language requirement, no minimum percentage, no entrance qualification beyond school completion.
- Your competition category is not the one you might assume. Indian students applying from India are classified as non-EU resident abroad. Under the decree, candidates in this category compete only within the seat quota of the single university where they filed their pre-enrolment application, rather than in the national ranking. That makes the choice of university, made months before the exam, more consequential than the score itself in some cases.
- NEET remains mandatory for India. The National Medical Commission requires NEET-UG qualification before joining a foreign medical course for you to sit FMGE or NExT and register in India afterwards. Italy will not ask for it. India will.
