Doctor Salary in Netherlands 2027-28: The Real Monthly Range by Career Stage
Doctor salary in Netherlands figures published in English, are unusable. Glassdoor’s Netherlands page reports an average of โฌ4,367 per year and, in the same block, a highest salary of โฌ4,600 per year. ERI reports โฌ145,281 nationally while reporting โฌ202,492 for Amsterdam, a city figure 37% above its own national average.
None of it is necessary, because the Netherlands publishes negotiated pay tables. Dutch doctors sit on collective agreement scales with named steps: the Cao Ziekenhuizen for general hospitals, the Cao UMC for academic centres, the AMS for employed specialists, and the Cao Huisartsen in loondienst for salaried GPs.

The specialist band is the one worth showing the working for. The official AMS salary table published by the LAD and compiled by NVZ Werkgeverszaken sets the seven-step scale at โฌ8,192 to โฌ14,219 gross monthly as at 1 June 2024. The LAD then agreed an 8% rise with the NVZ in four 2% steps through August 2026, which compounds to โฌ8,868 to โฌ15,392. That is where a specialist entering 2027-28 starts and finishes.
Salaried GPs are also rising: the LHV, InEen and LAD agreement runs to 31 December 2027 with a structural 8.25% increase, 3.5% from August 2026 and 4% from January 2027, so expect the GP figures above roughly 7 to 8% higher across our year frame.
Counselor insight: Search the Dutch terms rather than English estimators: "AMS salaristabel", "cao ziekenhuizen salarisschalen", "cao huisartsen in loondienst". The real tables take ten minutes to find. English results are poor because Dutch doctor pay is negotiated, not a market price, so there is no survey worth collecting.
Why Your Doctor Salary in Netherlands Is Higher Than Twelve Monthly Payslips
Every English result reports annual pay as twelve times monthly. Dutch employment does not work that way. Holiday allowance of 8% is standard, paid as a lump sum in May or June, so a โฌ8,868 monthly specialist salary is worth about Rs.1.27 crore (โฌ114,929) a year rather than Rs.1.18 crore. An ANIOS on โฌ3,000 is on roughly Rs.43.1 lakh (โฌ38,880) annually, and an AIOS at the bottom of the training scale on about Rs.61.4 lakh (โฌ55,430).
The Cao Ziekenhuizen also pays a year-end payment on top, widely reported at 8.33%, which would add a further month of salary.
Then the part nobody mentions. Under the Cao UMC, an AIOS has a standard working time of 2,392 hours a year, which is 46 hours a week. Specialists under the AMS have a 45-hour standard week excluding on-call shifts, and may average up to 52 hours including them, occasionally reaching 55. Any hourly rate you have seen for a Dutch doctor was calculated on 40 hours and is therefore overstated. Specialists do get 30 vacation days a year at full time under the AMS.
On tax, the expat ruling matters and is changing. The maximum tax-free allowance drops from 30% to 27% for new rulings from 1 January 2027, which falls inside this academic year. One clause is genuinely useful for doctors: medical specialists in training are automatically eligible for the ruling regardless of whether they meet the salary norm, unlike most other applicants.
Counselor insight: When you compare a Dutch offer against a UK or Gulf one, convert to an hourly figure using the real contracted hours from the cao, not 40. A 46-hour AIOS year changes the comparison materially, and it is the number that decides whether you can also study for anything alongside the job.
Can an Indian Doctor Legally Earn a Doctor Salary in Netherlands?
This is where the honest answer diverges sharply from the search results. You cannot practise medicine in the Netherlands without registration in the BIG register. An Indian MBBS is a non-EEA diploma, which means you cannot register on the strength of the degree. You must first complete the Verklaring van vakbekwaamheid procedure, run by the CIBG, an agency of the Dutch Ministry of Health, with your qualifications assessed by the Commissie Buitenslands Gediplomeerden Volksgezondheid.
The language requirement is the real gate. Doctors must show Dutch at B2 level or above, with demonstrable medical language ability, plus English at B2. The BIG register states plainly that learning Dutch to the required level takes on average one and a half to two years. Note what that means in practice: your clinical career is on hold while you learn a language to professional medical standard.
Two further points that catch people out. The BIG register puts the procedure itself at six months to eighteen months from the date you submit your application, on top of the language years. And registration only ever covers the basic qualification of basisarts. Recognition of an Indian specialisation, if you hold one, is a separate procedure afterwards with the relevant registration commission.

The BIG register publishes the examination contribution as โฌ1,700 for doctors, against โฌ1,500 for dentists and โฌ400 for nurses. Submitting the application and the commission's assessment of your file carry no charge. Everything else depends on your documents and your language route.
The real cost is not on this table. It is 18 to 24 months of full-time language learning during which you are not practising medicine, followed by a procedure of six to eighteen months, followed in some cases by required additional training. Budget in years, not euros.
Counselor insight: Do not pay a consultancy a large fee to "handle BIG registration". The application and the assessment are free, the fees are published, and the two things that actually determine your outcome are your Dutch and your curriculum documentation. Neither can be outsourced.
When the Doctor Salary in Netherlands Route Goes Wrong
The CBGV finds your curriculum not equivalent. This is a common outcome for Indian degrees, and it means required additional training rather than a refusal. Ask for the specific deficiencies in writing so you can address them precisely.
You fail the beroepsinhoudelijke toets. Prepare for it as a Dutch clinical examination, not an Indian one. Dutch practice guidelines, referral norms and the GP-gatekeeper structure differ substantially from what you trained in.
Your Dutch plateaus at B1. Very common, and the gap from B1 to B2 with medical vocabulary is the hardest stretch. Immersion in-country beats classroom hours in India by a wide margin.
Your permit runs out during the language years. This is the most frequent practical failure. Do not begin the language phase without a residence basis that covers the whole of it, and count the procedure months too.
Your offer falls below the IND threshold. An ANIOS salary can sit under the standard under-30 criterion. Check the threshold that applies to your age and graduation date before you accept, not after.
You return to India instead. Entirely reasonable, but if you qualified abroad you will need to clear the Indian screening examination to practise at home.
