What is IMAT Exam and Who Takes it
The IMAT exam, or International Medical Admissions Test, is the single entrance test for English-taught Medicine and surgery, dentistry, and veterinary medicine degrees at Italian universities. It is run by Italy’s Ministry of University and Research (MUR), and under the 2026 ministerial decree the paper is prepared by CINECA, the ministry’s in-house consortium. Many websites still credit Cambridge Assessment, which was true in earlier years and is no longer accurate.
Three degree classes sit behind this one test: Medicine and Surgery (LM-41), Dentistry (LM-46) and Veterinary Medicine (LM-42). At registration you must declare whether you are competing for Medicine plus dentistry or for veterinary medicine, and that choice becomes irrevocable once registration closes.

Worth clearing early: Italy has reformed admission to its Italian-taught medical courses through an open-semester system, but the 2026 decree explicitly excludes English-taught courses from that reform. The degree is a six-year single-cycle program, which is what Indian students usually mean by MBBS in Italy.

IMAT Exam 2026 Dates and Deadlines
The IMAT exam 2026 will be held on 29 September 2026, starting at 13:30 Italian time in Italian centres, with the start time adjusted to local time zones at overseas centres.
This date changed twice. MUR first announced 8 October, corrected it to 30 September, and the decree of 6 August 2026 fixed it at 29 September. Many coaching sites still display the older dates.

IMAT Exam Pattern, Syllabus and Scoring
The IMAT exam is a paper-based test of 60 multiple-choice questions to be answered in 100 minutes. Each question has five options and exactly one correct answer.

Scoring is unforgiving: 1.5 marks for a correct answer, minus 0.4 for a wrong one, zero for a blank, out of a maximum of 90. The 2026 IMAT exam syllabus is identical to 2025 and 2024, so papers from those years are your most reliable practice material.
Ties are resolved in a fixed order: section scores in biology, chemistry, physics and maths, logic and reading, then possession of an English language certificate, then the younger candidate.
Counsellor insight: With five options and a 0.4 penalty, a blind guess has negative expected value. Answer only when you can eliminate two options. Students trained on NEET habits over-attempt and finish below their true level.
IMAT Exam Eligibility for Indian Students
What Italy actually asks for:
- A completed Class 12 qualification to enrol
- Registration on Universitaly within the official window
- Nothing else: no separate application form, no interview, no statement of purpose, no Italian language requirement
How Indian students are classified:
- EU candidates and non-EU candidates resident in Italy need at least 20 points to enter a ranking
- Applicants from India fall into a separate category, non-EU resident abroad, where that threshold does not apply
- You need only a score above zero and at least one attempted question
- That sounds generous and is misleading, because the seats you compete for are far fewer
The NEET question:
- Italy does not ask for NEET, and Italian universities do not look at it
- India does. Under the National Medical Commission's Foreign Medical Graduate Licentiate Regulations, 2021, an Indian citizen must have qualified NEET-UG before joining a foreign medical course
- Without it, you cannot later sit FMGE or NExT and register as a doctor in India
- The NMC states this on its guidance page for students studying abroad, and confirms the result stays valid three years for this purpose
One clause worth knowing:
- The decree treats anyone who studied at least one school year in an English-medium school as automatically holding a language certificate
- For most CBSE, ICSE and state-board students from English-medium schools, that tiebreaker is already covered
- It is self-declared and can be verified, so declare it honestly
Counsellor insight: Skipping NEET does not stop you studying in Italy. It permanently stops you practising in India. Even students planning to settle in Europe sit NEET, to keep the door home open.
How Indian Students Register for the IMAT Exam
IMAT exam registration happens only on the Universitaly portal at universitaly.it. There is no alternative channel and no agent who can do it elsewhere. Your name, date of birth and document number must match your passport character for character. The 2026 fee appears on Universitaly when registration opens; see the cost breakdown below. Payment alone does not register you: without the online receipt, you cannot sit the test.
Pre-enrolment is a separate, earlier and more important step. It happens on Universitaly under the international students section, it makes your study visa possible, and each university sets its own deadline. Several 2026-27 windows have already closed: Sapienza on 30 June, Messina on 20 July, Milan on 31 July, and Catania at its cap of 560 applications. If you have not pre-enrolled anywhere, plan for 2027-28 rather than paying for a test you cannot convert into a seat.

Counsellor insight: The most common failure we see is a declined card on registration day. Indian banks disable international transactions by default, so call your bank a week ahead.
Is the IMAT Exam the Right Route for You
If you are in Class 12 now and appear for NEET 2027: you are in the strongest position. Sit NEET as planned and use the window from boards to August 2027 for preparation. Begin apostille and CIMEA work by April 2027, and pre-enrol as soon as your target university opens. Biology and chemistry overlap heavily with NEET; the logic section does not, and that is where you will need fresh practice.
If you have qualified NEET but missed a government seat: Italy is a real alternative to a private Indian college costing several times more. Your NMC compliance is already handled, so the work is preparation and documentation. Be realistic about which universities hold non-EU seats in double digits.
If you have not pre-enrolled anywhere for 2026-27: you may still register and sit the test on 29 September, but with no valid pre-enrolment there is no local ranking to place you in. Treat it as a diagnostic attempt if you wish, and build a proper 2027-28 plan.
What to Do When Things Go Wrong
You missed registration. There is no second session and no late entry. The 9 September deadline is absolute under the decree, and your next opportunity is 2027.
You missed pre-enrolment. Check whether any university still has an open window, since deadlines are set individually. If none are open, your score cannot become a seat this cycle.
Your score is low. Scores are valid only for the year you sit them, so a retake is a full retake. There is no attempt limit, and many admitted students are on a second attempt.
You are disqualified on the day. The decree voids papers for phones, smartwatches, tablets, earphones, your own pens or stationery, notes, interaction with other candidates, and signing the answer sheet. Carry valid photo ID; without it you cannot enter.
Your visa is delayed. The outer deadline for 2026-27 visa applications is 30 November 2026. Apply the moment you are assigned a seat, keep your Universitaly summary and admission letter, and write to your university if the appointment slips.
