12 min read

Quick Read

  • PLAB Exam has two parts: PLAB 1 (MCQ) and PLAB 2 (clinical OSCE in Manchester).
  • PLAB 1 fee is Rs.35,941 (£283); PLAB 2 fee is Rs.1,31,572 (£1,036) from April 2026.
  • IELTS Academic 7.5 overall (7.0 in each section) is mandatory to register.
  • PLAB 1 can be taken in India before completing the internship.

What Is the PLAB Exam and Who Needs It?

PLAB Exam Full Form is Professional and Linguistic Assessments Board, which is the main licensing path for international medical graduates (IMGs) to get GMC registration. It is the easiest option to practice in the UK for Indian MBBS graduates without a postgraduate qualification recognized by the GMC.

PLAB 1 is a written, multiple-choice paper testing clinical knowledge across all major specialties. PLAB 2 is an Objective Structured Clinical Examination (OSCE) held only in Manchester, where you demonstrate history-taking, communication, and clinical decision-making in simulated patient scenarios. Both parts are mandatory. After passing PLAB 2, you have two years to apply for GMC registration before results expire.

Counselor insight: If you are a graduate with a recognised postgraduate qualification, for example MRCP, MRCS or MRCOG, then you may be eligible for a specialized registration option that avoids the PLAB Exam entirely. PLAB is the only way out for most of the MBBS only graduates from India. Please confirm your eligibility with the GMC before assuming any exemption.

PLAB Exam Eligibility for Indian Doctors in 2026

Eligibility RequirementSpecifics for Indian Applicants
Primary Medical Qualification (PMQ)MBBS from a college listed in the World Directory of Medical Schools and recognised by the GMC
English Proficiency: IELTSIELTS Academic overall score of 7.5, with a minimum of 7.0 in each section. IELTS Indicator and IELTS Online are NOT accepted
English Proficiency: OETOET Medicine, with a Grade B in all four subtests, is a valid alternative to IELTS. Valid alternative to IELTS
English Test ValidityMust be within two years of your PLAB 1 application date
InternshipNot required to sit PLAB 1; 12-month verified internship required for full GMC registration
Attempt LimitMaximum 4 attempts per part; a fifth requires evidence of additional postgraduate training
Age LimitNo official ceiling; practical minimum around 23-24 years given degree and internship duration

IELTS scores from a one-skill retake are not accepted. All four band scores must be achieved in a single sitting.

Counselor insight: The GMC verifies IELTS and OET results directly with the exam bodies, which takes about two working days. Do not book a PLAB 1 date before your English evidence is verified in your GMC Online portal; an unverified account will block the booking option entirely.

PLAB Exam Pattern: PLAB 1 and PLAB 2 Explained

PLAB 1

PLAB 1 is a 3-hour written exam with 180 single best answer (SBA) questions, based on the GMC's MLA content map updated in August 2024. Topics span internal medicine, surgery, obstetrics and gynecology, pediatrics, psychiatry, and emergency medicine. All clinical scenarios follow UK protocols, primarily NICE guidelines and the BNF, which differ meaningfully from Indian textbook sequences, especially in first-line treatment choices and referral pathways.

Pass mark: 115-120 out of 180 (roughly 60-65%). Overall pass rate: 65-75%. The pass mark adjusts per sitting based on question difficulty.

PLAB 2

PLAB 2 has 16 clinical stations of 8 minutes each, testing history-taking, physical examination, communication skills (breaking difficult news, consent), and data interpretation. According to GMC's published pass rate data, the PLAB 2 pass rate has ranged between 58% and 66% in recent sittings, making it the harder of the two parts for most IMGs. It is held exclusively at the GMC's clinical assessment centre in Manchester; there are no international venues.

Counselor insight: Indian candidates often score well on clinical knowledge in PLAB 1 but lose marks on "most appropriate next action" questions. These reward the UK-standard first step, not the most thorough investigation. Train yourself to think in NICE protocols, not Indian textbook sequences. For PLAB 2, no amount of reading substitutes for live OSCE station practice with a partner.

PLAB Exam Preparation: What Works for Indian Doctors

Key Resources

For PLAB 1:

  • Oxford Handbook of Clinical Medicine (OHCM) foundational UK clinical reference
  • PLABable and Passmedicine the most-used SBA question banks for PLAB 1
  • Fastest solid alternative with detailed answer explanations

NHS Salary After PLAB 2026: What Indian Doctors Actually Earn

Most Indian doctors who clear the PLAB Exam enter the NHS at Foundation Year 2 (FY2) or as Senior House Officers in specialty training. Salaries below are from the NHS Terms and Conditions of Service handbook, 2025-26 pay award, published by NHS Employers. Pay scales are reviewed annually each April. Verify the current rate on the NHS Employers page before finalizing your financial plan.

GradeAnnual Salary (GBP)Annual Salary (INR Approx.)
FY1£36,616Rs.46.5 Lakh
FY2£43,000Rs.54.6 Lakh
Specialty Registrar (ST1-ST2)£50,000-£58,000Rs.63.5-73.7 Lakh
Consultant£1,05,504+Rs.1.34 Crore+

These are base salaries. On-call allowances, night shifts, and London weighting (£2,000-£3,000/year extra) increase take-home significantly. Exchange rate used: Rs.127 per GBP.

Counselor insight: A GMC number does not guarantee an NHS job. Specialty training posts are competitive, and many IMGs wait months between registration and their first post. Build your portfolio, audits, research, and presentations before you arrive. Start searching NHS Jobs as soon as your PLAB 2 result is confirmed.

What to Do If Things Go Wrong

If You Fail PLAB 1

You have up to four attempts. After four failures, a fifth requires evidence of additional postgraduate training or clinical experience submitted to the GMC. A sixth attempt is not possible. Check your GMC Online result breakdown, identify the weak subject clusters, and switch question banks if your current approach is not working.

If Your IELTS Score Misses 7.0 in One Section

One-skill retake certificates are not accepted. You must resit the full IELTS and hit 7.0 in all four sections in a single sitting. If writing is consistently below 7.0, consider switching to OET Medicine. Many Indian doctors find the clinical context of OET questions more familiar.

If You Miss the Two-Year PLAB 2 Window

Your PLAB 1 result expires, and you must resit it from scratch. This is a real risk when UK visa delays push back your Manchester date. Book PLAB 2 as soon as your PLAB 1 result arrives and apply for the UK Visitor Visa immediately after.

If Your UK Visitor Visa Is Refused

Reapply with stronger financial evidence and clearer documentation of ties to Indian property, family, and employment. Apply at least 10-12 weeks before your PLAB 2 date to allow time for a reapplication if needed. UKVI official guidance is the only reliable source for current visa requirements.

Three Takeaways Before You Begin the PLAB Exam Journey

  1. Start with IELTS, not PLAB registration. Your GMC account will not unlock the PLAB booking option unless you have a verified IELTS Academic score of 7.5 overall and at least 7.0 in each section. Every other step waits on this requirement.
  2. Budget Rs.4-7 Lakh for the full journey, not Rs.1.67 Lakh. The exam fees are the smallest part. PLAB 2 requires a trip to Manchester, a UK visa, and a 1-2 week accommodation plan for these expenses before you begin.
  3. Build your NHS CV before you land. GMC registration is essential, but specialty training posts are competitive. Start audits, presentations, and any clinical research while preparing for PLAB; it directly affects how quickly you get your first NHS post after arriving.

Frequently Asked Questions About PLAB Exam

  • Is PLAB harder than USMLE?

PLAB is tougher than USMLE. PLAB is believed to be easier to pass. USMLE has three levels, takes 3-5 years to complete, and needs US residency matching, which is highly competitive for IMGs. PLAB comprises 2 phases. Most Indian doctors get NHS jobs within 12-18 months of starting the process.

  • What is the salary of a doctor in the UK after PLAB?

After clearing the PLAB Exam and gaining GMC registration, most Indian doctors start at Foundation Year 2 (FY2), earning approximately Rs.54.6 Lakh/year (£43,000) as base pay. Specialty registrars earn Rs.63.5-73.7 Lakh/year, and consultants earn Rs.1.34 Crore+ all before on-call and shift allowances.

  • Which is tougher, NEET PG or PLAB?

Both test different things. NEET PG is a high-volume MCQ race with extreme cut-throat competition among Indian graduates. PLAB 1 is manageable with 6-8 months of focused preparation; PLAB 2 is harder because it tests live clinical communication in UK-style patient scenarios, which most Indian medical colleges do not train for.

  • Can I work as a doctor in the UK without PLAB?

Yes, in limited circumstances. If you hold a GMC-recognized postgraduate qualification such as MRCP, MRCS, or MRCOG, then you may qualify for the CESR (specialist registration) pathway and avoid PLAB altogether. Doctors in certain research or non-clinical roles may not need to be GMC registered. PLAB Exam is necessary for direct NHS clinical practice as an IMG with an Indian MBBS alone.

  • Is giving PLAB Exam worth it?

For Indian MBBS graduates targeting the UK, yes. The PLAB Exam costs Rs.4-7 Lakh end-to-end and leads to a structured NHS salary starting at Rs.54.6 Lakh/year with a realistic 12-18 month timeline from beginning to first post. Compared to USMLE or staying in India's competitive PG system, the return on investment is strong.

  • Is PLAB Exam valid in India?

No. Passing the PLAB Exam gives you GMC registration, which is a UK-only license. It has no legal standing with the Medical Council of India and does not permit you to practice clinical medicine in India. If you return to India after UK practice, you would need to re-register with the NMC (National Medical Commission).

  • How much money is required for PLAB Exam?

Exam fees alone are Rs.1,67,513 (PLAB 1 + PLAB 2 combined from April 2026). Adding IELTS preparation, question bank subscriptions, UK Visitor Visa, return flights to Manchester, and two weeks of accommodation, the realistic total budget is Rs.4-7 Lakh more if retakes are needed.

  • What to do after clearing PLAB?

Apply for GMC registration within 2 years of passing PLAB 2, along with your internship certificate, MCI registration, and certificate of good standing. Once registered, you can apply for NHS vacancies on NHS Jobs and get a Health and Care Worker Visa so you can start working in the UK.

  • How long is PLAB valid for?

PLAB 1 is valid for two years. You must sit and pass PLAB 2 within that window, or you will need to retake PLAB 1. After passing PLAB 2, you have another two years to apply for GMC registration. There is no expiry on GMC registration itself once granted, as long as you maintain your license through annual revalidation.

  • What is the success rate of PLAB 1?

The historical pass rate for PLAB 1 has been in the range of 65 to 75 percent. After the MLA alignment sittings from August 2024 onwards, the pass rate was in the lower band of 55-65% during the transition period, but it is likely to stabilize as candidates become more familiar with the new MLA content map (GMC PLAB pass rates). The major variable remains the quality of preparation. Candidates that use the MLA mapped question banks and practice clinical reasoning are routinely outperforming those using pre-2024 PLAB materials.

  • Is there any age limit for PLAB?

The GMC sets no official upper age limit for the PLAB Exam. The practical minimum is around 23-24 years, given the time needed to complete an Indian MBBS degree and the required 12-month internship before full GMC registration. Doctors in their 30s, 40s, and beyond regularly sit and pass PLAB.
