I am now completing a 3-year MD in the Ayurvedic specialisation of Panchakarma at the Kerala University of Health Sciences affiliated institution VPSV Ayurveda College, Kottakkal. Briefly, the BAMS, Bachelor of Ayurvedic Medicine and Surgery degree is an undergraduate degree with a course duration of 5.5 years - 4.5 professional years and 1 year of internship at an Ayurvedic hospital. The course covered the fundamental concepts in Ayurveda and also covered modern medicine counterparts such as human anatomy, physiology, pathology, pharmacology, toxicology, general medicine, ophthalmology, E.N.T, paediatrics, gynaecology, and obstetrics. The one-year internship provided a period of intensive clinical experience. I wish for a future in the young but growing area of integration of modern and Ayurvedic sciences and further develop robust research evidence for such integration. The MRes clinical research course description excites me because I presume that the course will provide me with the research skills and prepare me for my planned future - a PhD degree and a life in research and academia needed for building up the research base for Ayurveda. Finishing high school with majors in physics, chemistry, biology, and maths, making a radical epistemological shift to Ayurvedic science was challenging but immensely satisfying. However, after a few days, the deficit in the present healthcare system hit me hard. Having completed 2 degrees and internships at both ayurvedic and allopathic hospitals, I understand the lacuna in both streams of medicine, their potential scope and the fact that there is very little communication between the two systems of medicine. To satisfy the emerging needs of the society, the strengths of both systems of knowledge should be put together and for that, I realize there is a need to develop a common understanding of these two epistemological branches of medicine. This is especially important because our current understanding of biomedicine has limitations and falls short of society's needs in terms of health. Take the human genome project as an example. Due to the huge combinatory potential of the genome, the study was unable to have a significant impact on personalised medicine. The need to stratify the healthy population became a necessity. Later, the concept of Prakriti in Ayurveda was integrated into the contemporary understanding of genomics to develop a new branch of science called Ayurgenomics. Stratification of the population became much easier with Ayurgenomics which is now considered a promising tool for personalized medicine. Also, in the context of drug discovery, we observe that the rate of new drugs discovered is decreasing, the number of drugs being withdrawn is increasing and there is an innovation deficit faced by the industry. The complexity of the discovery process, the length of time required, and the cost are further ongoing concerns in drug discovery. The large discovery space of molecules complicates the situation further. In light of this, research involving the reverse pharmacological approach of traditional time-tested medicines would be advantageous since they accelerate the drug discovery process. Significantly less time and money are required. In an era where the trend is shifting from single target drugs to synergistic formulations, the role of Ayurvedic medicines will prove to be inevitable. These examples demonstrate the important role that Ayurvedic science may play in medical advancements in the future. Therefore, the need of the hour, I realize is to find a voice that unifies the two epistemologically distinct branches of medicine. To be able to do this I would require the inquisitive mind of a modern- day researcher along with the ayurvedic understanding. Having acquired formal education for over 8 years in Ayurvedic science, I’m now looking for opportunities to engage in clinical research. Dr --- ---'s admission into the MSc evidence-based medicine degree at Oxford University in 2022 provided me with the impetus I needed to apply to overseas universities. Also, as I already have a research background, I was looking for research-based programs rather than the taught masters, which led me to MRes Clinical Health research degree. Specialized research departments, as well as the different partnerships and collaborations established by university of Birmingham, drew my interest to the university from the start. The multidisciplinary nature of the studies being conducted there made me feel that the institution would be ideal in providing an atmosphere where traditional systems of medicine like Ayurveda would be encouraged. Also, the research facilities of the university as I understand are one of the finest in the world and hence the course is quite tempting to apply for. My internships at both allopathic and Ayurvedic hospitals provided me with valuable clinical experience and improved my patient communication skills. I have also been observant of the hospital administration and functioning during these internships. My MD enabled me to learn the fundamentals of research, such as the different research methodologies, drafting of a research protocol, the ethical considerations associated with research, etc. As a part of my MD, I’m doing a research project on a specialised ayurvedic intervention (Udvartana), for the management of the dominant negative sensory symptoms in diabetic peripheral neuropathy. While working on the research project, I learnt the basic skills of clinical research, such as systematic case recording, doing a literature review, and writing the dissertation. To gain first-hand experience with various treatments and the making of different medications, I used to volunteer in our Ayurveda hospital's treatment rooms and medicine preparation facilities during the study period. I participated actively in competitions to develop my speaking and presenting skills, and I even managed to win a few prizes for my paper and poster presentations. I have also been a recipient of the “Ayurvisharadha award” by the Himalayan Drug Company for academic excellence. I focussed on my writing skills when I started writing blogs for an online hub on Ayurvedic nutrition. The experience of blog writing made me realize how difficult it was to translate the words in Sanskrit to English/regional languages. The exact translation sometimes wouldn’t convey the depth and breadth of the concepts. This also made me realize the need for adopting and validating contemporary terms for those mentioned in ancient Ayurvedic texts without diluting the essence and spirit of their concepts. As I gained confidence in my writing skills, I started focusing on writing scholarly articles. I have written a book review and a case report so far, both of which are under review. Being a school topper in classes X, XI, and XII, exams didn’t bother me much. I maintained a top 5% academic ranking throughout my undergraduate and graduate studies. My academic results won't be of value to me if I can't contribute to the development of the science of Ayurveda. By engaging in research, I will be able to build a strong evidence base for Ayurveda, which is much needed for the advancement of the science. From the MRes Clinical health research course, I hope to gain the competence needed to organise and perform methodologically sound research. More concepts, such as the Ayurveda biology proposed by Dr ---.--- ---, should be promoted. This will only happen if people who understand Ayurveda enter the realm of research. Furthermore, I anticipate that the training will prepare me to engage with an integrated research team and contribute Ayurvedic perspectives. This would also enable me to represent Ayurveda at a global level, especially at WHO-established traditional medicine centres.
