No, you do not have to take only IELTS for admission to German universities.
Many universities also accept other English proficiency tests such as TOEFL iBT, Cambridge English (C1 Advanced or C2 Proficiency), or the Duolingo English Test. Some may accept a Medium of Instruction (MOI) certificate if your previous studies were entirely in English.
