What documents are required to apply for a passport?

- Proof of identity (Aadhaar card, PAN card, voter ID, or driver’s license)
- Proof of address (utility bill, bank passbook, rent agreement, or employer certificate)
- Proof of date of birth (birth certificate from municipal authority or Registrar of Births and Deaths)
- Recent passport-sized photographs (as per Passport Seva Kendra specifications)
- Annexure or declaration forms (if required)
- Previous passport (if renewing; original plus copies of first two and last two pages)
