The faculty and staff accommodation form should be completed and sent to Human Resources, either in person or electronically. Emailing the form to strammel@latech.edu is the best way to return it.
Human Resources will determine whether more information is required to support the condition stated and will ask the employee to supply it.
The form is delivered to the appropriate dean or division head, with any necessary redactions for medical information protection.
