Requesting an accommodation

When seeking an accommodation, adjustment, or modification, it is the obligation of individual students, staff, and faculty to identify themselves as a person with a disability. Individual students, employees, and professors must also get documentation from a licensed professional demonstrating how their handicap impairs their ability to perform essential job tasks or participate in university activities or services. Medical records will be kept private in accordance with legal requirements and ISU policy.
